What are the benefits of the American Heart Failure Certification for patients?

The accreditation helps patients identify facilities that follow the latest evidence-based cardiology guidelines for heart failure care. This helps ensure coordination of pre-admission, during treatment, and post-discharge follow-up care.

What is the Skilled Nursing Facility Heart Failure Certification Eligibility Criteria?

  • Uses a standardized method of delivering services based on current evidence-based AHA guidelines
  • Adherence to evidence-based requirements aimed at improving care and outcomes for heart failure patients

Does every Skilled Nursing Facility go through certification to provide cardiac care?

Not every SNF goes through certification to provide cardiac care.
