Each community offers an array of amenities and services to enhance one's rehabilitation experience. Some services/amenities have additional fees.

Concierge Services

Each community is honored to introduce you to their accomplished nurses, whose careers have spanned all facets of long-term care, hospice, home care, and every level of hospital care.

An Added Level of Communication & Support

• Answer Questions via a dedicated phone line & email
• Provide Labs Review Orders
• Review Clinical Concerns
• Serve as Administrative Liaison to the Clinical Team

Our dedicated concierge nursing team is specially trained to bridge the communication gap between families, patients, and the clinical care team, ensuring every concern is addressed with the highest level of professionalism and care.

Family Matters Program

The Family Matters Program at each of the communities aims to alleviate some of the worry and concern you may be experiencing while allowing you to focus on what matters most… your loved one.

• Virtual Care Navigation Meeting – Understand what to expect of your loved one's stay via a virtual meeting with the interdisciplinary care team.
• Virtual Visitation – Stay connected via routine Skype and/or Facetime visitation with your loved one.
• Life Loop – Keep in the loop with your loved one's engagement in activities and life-enriching programming via a shared web-based application.
• Virtual Home Evaluation – Receive consultative advice from members of the therapy team on how to best prepare your home for your loved one's safe return.
• Guest Service Line – Access a member of the community's management team for any experience enhancement needs at any time via their established Hospitality Line.
• Primary Care Notification and Follow-Up – The clinical team will notify and keep your loved one's PCP updated on admissions status and progress.
• Medical Director Monthly Call – Join a monthly call with the Medical Director to receive updates on current center practices with an open Q&A forum.
• Discharge Support – The community will assist in coordinating resources and provide complimentary dinners for the first two days at home.
• Outdoor Safe Visitation – Coordinated by the community's team.
